Student Registration Form for Enrolled Students

Before you begin, you will need to prepare the following:

• A head-shot picture of yourself
• A picture (.jpg, .jpeg, .png, .pdf) of your Bank Deposit Slip (if you paid over-the-counter at the bank) or of your Transfer to Anyone Confirmation (if you paid via the BPI Mobile App) or of a picture of your Acknowledgment Receipt (if paid through booth registration).

If uploading a picture of the Deposit Slip, please write (in ink) the name of the student at the bottom of the deposit slip. Make sure the name is legible and easily readable when you take the picture of the Deposit Slip.
